Bring it with you so the Scoutmaster can sign it when you have your Scoutmaster Conference.
Bring this sheet (signed by the Scoutmaster) to your Board of Review.
Have the Board Members print their names and sign the appropriate line. They must indicate whether the Rank Badge is awarded or not, and date it.
After your Board of Review, please return this completed form to the Advancement Chairman so he can record it and get your Rank Badge for you.
